Continuum is the app your patients use to organise their own medical documents before they see you. What lands in your hands is not another stack of loose pages, but one clear page: current medications, a timeline of what changed, and the two or three questions they actually want to ask.
Ten minutes, sometimes less. A patient who has seen a specialist, had bloodwork twice, and started a new medication since the last visit, and who remembers the details approximately, if at all. A good part of the appointment goes into reconstructing what already happened, before you can get to what happens next.
